  • Application process:

    • PLLCs:

      • Certificate of Professional License is required

        • Must mail cash or check made out to: North Dakota Board of Medicine and mail to:

          4204 Boulder Ridge Rd

          Suite 260, Bismarck ND 58503

        • Please indicate in the mailing that you request the letter is sent via email back to you.

    • PCs:

      • Form SFN 13610 is required

      • Mail the completed form, along with a $30 check made out to North Dakota Board of Medicine:

        • 4204 Boulder Ridge Rd

          Suite 260, Bismarck, ND 58503-6162

  • Once the document has been obtained, upload it as a PDF to your Discern application under the Verification of License Document upload:
